Packaged in foldover, double sided grayscale sleeve.  The Rikk Agnew credits are entered exactly as they are listed in liner notes. There is also a credit for the Larry Flynt Marching Band.  That credit was not added to the tracks, as it is a fictitious credit, as are the instruments listed above.
